We are having file system issues with a physical server running Windows Server 2008 R2. It is patches with all latest updates.
It has the following drives/volumes:
- Disk 0 - 2TB SAS RAID1 disk with F: volume
- Disk 1 - 300GB SAS RAID1 disk with C: and System Reserved (V:)
- Disk 2 - 2TB SATA non-RAID with H: volume (utility disk)
- Disk 3 - 2TB SATA RAID1 G: volume for internal backups.
- Disk 4 - 8GB USB 3.0 thumb drive with K: volume (Bitlocker key for backups to J: volume)
- Disk 5 - 2TB USB 3.0 external drive with Bitlocker J: volume.

Every few days our backups fail - saying the backup target is not available. We also get messages like the following:
- Event 3: (Filter Manager) Filter Manager failed to attach to volume '\Device\Harddisk7\DR13'. This volume will be unavailable for filtering until a reboot. The final status was 0xc03a001c.
- Event 15: (Disk) The device, \Device\Harddisk7\DR13, is not ready for access yet.
- Event 9: (Virtual disk Service) Unexpected provider failure. Restarting the service may fix the problem. Error code: 8007001F@02000014
- Event 85: (volsnap) Volume J: is offline for shadow copy protection. A read failure occurred during a shadow copy on write operation. Please try clearing the protection fault or restart the computer. If all else fails, revert out of shadow copy protection mode to reclaim the use of the volume while losing the shadow copies.
- Event 57 (Ntfs) The system failed to flush data to the transaction log. Corruption may occur.
- Event 141 (Ntfs) follows and mentions the J: drive which is our backup drive.
The J: drive seems to be accessible in Windows Explorer - i.e. files can be listed - but the backup says it's not accessible and the Properties dialog shows 0 bytes. Disk Manager and/or diskpart also show the drives with 0 bytes. Sometimes they show additional drives with not attributes that are also not accessible.
We have run chkdsk on the internal drives and the external backup drive, updated drivers, restarted the VDS service and any other thing we can think of. We've r
If we reboot the server all will be fine for a while - then the problem comes back again.
One other thing we notice is the output of mountvol shows "lots" of volumes with "NO MOUNT POINTS". In the past we might see a few - I thought that was just a stale entry from either swapping external backup drives or from VHD's that were mounted during the backup process. But now - we see 50-60 volumes show witih "*** NO MOUNT POINTS ***". At the end of the note is an example listing - a lot of the mount point ids seem very similar, but not exactly the same. Also, at the point this was taken, we are not seeing problems with accessing either any internal or external drivers.
Any ideas or thoughts on how to solve this are appreciated.
